What you can do is add a checkbox that prompts the user if they would like to personalize and if selected add a "personalization fee" product to the cart.

There is an example here: http://bouncingfrogs.com/collections/blankets/products/baby_blanket_chloe

Another approach is to create a variant that represents the personalized version. If the checkbox is selected, set the form's id field to be that variant instead of the default. I think this is a cleaner approach, but not feasible if you are using product options.
